Which of Gerrard, Scholes and Lampard would've been the designated
"left winger"?

And which one would've been the "holding" midfielder?

Neither combination would've shocked me really.

He did in Euro 2004. You'd need an England fan to tell you whether or
not it was successful, but I vaguely recall Scholes starting on the
left vs. France and seeming aout of place.
