I agree with the others on here - a football signed by you all would be the perfect gift for your coach. Not only is it a personal memento, but - as you say he also taught you basketball - you are also expressing your thanks for the sport in which you consider his input as most important.
I know that you stated that you would like to give him a personal gift, but organising something from the team would show that he has created a good team ethic amongst his students and that they all appreciate him - any teacher would be touched by an acknowledgement of their hard work, and good teachers should be recognised and remembered, as they are the kind that rarely do it for personal reward.
If you want to add a personal note to your gift - when you sign your card, make sure you write the sentiments that you have expressed on here - he is obviously a good coach to bring out such positive feelings in you and it would be nice for him to hear what a difference he has made to you.
